Efficacy/Effect 1. Decreases ammonia concentration in cases of acute exacerbation of hyperammonemia due to the following diseases, which cannot be controlled by oral administration
1) Congenital urea circuit abnormality
- Carbamyl phosphate synthetase (CPS) deficiency
- Ornithine transcarbamylase (OTC) deficiency
- Arginosuccinate synthetase (ASS) deficiency
- Arginosuccinate lyase (ASL) deficiency
2) Lysinuric protein intolerance
2. Acidifying agents in severe metabolic alkalosis
Usage/Dosage 1. Decreases ammonia concentration in cases of acute exacerbation of hyperammonemia. To be used only when control is impossible with oral administration
- Continuous intravenous injection of L-arginine hydrochloride (0.2∼1g per kg of patient body weight) over 1 hour
2. Acidifying agents in severe metabolic alkalosis
- Dose of intravenous injection: L-arginine hydrochloride (g) = the amount of decrease to reach in plasma carbonate concentration (mM/L) × [patient's weight (kg)/9.6]
3. Pituitary function test to measure growth hormone secretion
- Continuous intravenous injection of L-arginine hydrochloride (500mg per kg of subject body weight, 10% solution) is administered over a 30-minute period
- Diagnosis
1) Patient must fast and then go to bed at their normal time. Testing is carried out the next morning
2) Have patient lie down for at least 30 minutes before injection
3) After injection into the anterior cardinal vein or other appropriate vein, blood is drawn from a vein in the opposite arm
4) Blood is drawn 30minutes before the injection, right after the injection, then 30, 60, 90, 120, and 150 minutes after the injection
5) Administer the recommended amount for 30 minutes. Keep dosage constant throughout the injection
6) The blood drawn before radioimmunoassay is immediately centrifuged and the plasma is stored at -20℃
7) Growth hormone deficiency is diagnosed by retesting this drug or an insulin-induced hypoglycemia test
(Wait 24 hours before retesting)
